The Flow of Both Black Paper quests:
-> Use a Photon Crystal to access any monster pit
-> Defeat the monsters
-> After defeating all monsters 1 red box with the special weapon is left on the floor
-> Quest Finished
If you use a Photon Crystal and you die and you return to the City, you don't get the reward and also you lose the Crystal so be careful and bring many Fluids and Mates with you.
As for the auto announcement that appeared at the top with the Block+ Party Name + the Name of the item, this announcement was enabled only as a Special Event for x1, x5 or x10 increased rare rates on the official servers, it wasn't an ordinary system that was always enabled.

For the Karen EP4 Epilogue route in Pioneer Spirits you must clear both Warriors Pride (戦士の誇) and Restless Lion (蠢動の獅子)
Please verify carefully checking original quest guides before reporting anything as a bug

If you were just fighting the dragon and got 100 in the middle of the fight then it was your connection getting desynced from the server, try to play using wired connection if possible.
Other kind of Error 100. might be caused by server side bugs and such but those are very rare to find these days as i'm always hunting for them before any of you find them.
